Nothing should slip out and there are no acceptable error's it just happens when it should not and probably will continue just by simple or complicated uncalled for distractions when in the middle of project.

Two principles, one, that no error is acceptable and two, that errors will ALWAYS occur.

Ok, in manufacturing, companies work hard on the processes to prevent/detect errors and they are MEASURED in terms of occurrences per million.   

So the real issues, day to day, are when the inevitable occurs - what is the relationship between manufacturer and customer.

My brother in-law makes Windsor chairs by hand.  Guaranteed for life - as he says either his or yours.

The Japanese had/have a motto that my company picked up
especially for exports to Japan:
Zetsumo Ibutsu
Meaning  "Zero Defects"
We even had t-shirts to pass out to the plant and QC workers
with that motto on them.
We tried really hard to fulfill that principle

I would agree with this, as it pertains to large production companies. They move so much product that they are willing to live with a certain percentage of errors, and usually make those right if contacted. 
But I look at small businesses differently, or custom work differently. Since the products they supply are either custom, or limited numbers, it often means the owners are either working too, or very hands on, and see everything that goes out the door.
These small businesses or custom shops don't normally have margins of error, as they can keep a closer eye on what leaves the company.
