Fulghum rest made by Randolph Machine. A fantastic rest with belt drive elevation adjustment. Pricey, but well worth the money for me. (I bought mine at a huge discount because it had a very small flaw in the powder coated finish.)

I bought my belt on Amazon, but they show them out of stock. It should be available elsewhere.
