Problem with H&R Trapdoor

About 10 years ago I bought an H&R Trapdoor Officers Model off an online auction. I shot it a few times and then the breechblock poped open and a empty hull shot out. I looked at it, retightened a set screw and shot it a few more times and wham, it did it again. I put the gun away and moved on. Flash forward to last May. I dragged it out, set the screw with Loctite and tried it again. Same thing. One shot and it flew open. The loads are fine: 55 grains of 2F, 400 grain bullet.  It's the gun, the H&R mod to the Breechblock opening lever to be exact. I need a good smith. Any recommendations? Any experience with this problem? I do have an original breechblock but I'm thinking it will need to be hand fitted.

drill a detent where the set screw goes so it won't rotate open.

Cbashooter has the answer also place a second lock hex against the first as this will lock them together like double nutting a bolt.

Yes, you are correct I also did that. thank you for adding that. I did this to my own and 2 other friends rifles and the problem was solved.
Mine was an officer's and my friends had carbines.my officer's would occasionally shoot 5 shot groups into an inch at a 100 yd with smokeless loads. It was quite an amazing gun but I let it go.

This seems like an easy fix. I also have an original Breechblock so if this fix goes south, I have a fall back.

I had the same problem, sent the rifle to a gunsmith down south and it got fixed. I believe what they did was drill a detent into the latch so that the set screw will hold better. Butting two screws against each other as recommended above would provide added insurance. They are great rifles and to my knowledge this is the only defect, and it can be fixed. 
Unfortunately, the replica latch assembly is not designed the same as the original, and I do NOT believe original parts are interchangeable. However, once I got mine fixed I did not completely explore that option. If possible, that would definitely be a permanent fix.
