American Boy's Rifles by Jim Perkins has a few pictures of the Page-Lewis Model D "Reliance."  The bolt is not shown separated but you can get a general idea of how it looks, at least.

They made a version with a "thumb trigger," according to the pics.  One of those would be a Rara Avis, for sure.

A quick update: after watching various parts stores and E BAY for several months I found a bolt for a D model on E BAY. Several people here said that they would interchange, had to pay more for it than the rifle is worth but I took a chance and it works. Fired it today after at least 50 years sitting around. The personal history makes it worth the extra cost of the bolt, it's a rusty, beat up old gun and I think it's going to stay that way. But at least it shoots again. Thanks to all who replied.
