If your hole threads are good it would be great to find a replacement.

When it happened to me both the screw threads and the hole threads were damaged so I re-threaded for a slightly larger screw.

Have you tried straightening the screw?

I hope this doesn't end up hijacking this thread, but rkba2nd's comment raised a long-time unresolved question for me:  How do you straighten a screw without damaging the threads?

Hardware store, find a couple nuts that fit, screw them on end clamp in vise, push on screw.
You’ll get it pretty close

Measure the thread then buy the nuts from McMaster Carr. Best source I know obscure threads.

I would straighten with the arbor press. 3 Nuts, on ends and the high spot in small tool makers vise. Rotate & indicate high spot with a dial indicator & true using a very light touch.

No different than truing a large shaft just small.

That’s the proper screw, McMaster may sell the right screw, large selection. To get the proper head another matter but right threads wrong style head may still allow the mount to clamp.

Original replacement better solution.

Boats solution is the same I have used many times to get myself out of a bind, if you will. I have, over many years, accumulated a large collection of taps. I use brass rod and drill and tap and proceed as boats laid out. The brass precludes most , if not all thread damage, and has worked well for me when it was the most expedient if not the best way to salvage a part. For a replacement, you might try Parson's, or one of the screw makers, or, as skalkaho mentioned, possibly Steve Earle.

11/16" shank. Maybe 18-20 years ago I got ahold of the guy that bought out Unertl and bought a couple cross screws for my then Unertls that all were bent . Seems to be the one Achilles heel with these scopes when picking them up.....
