yup, unless you are left handed and shooting it with an armored face mask, full body armor and there's no one around you.  Pains me to see a Hepburn like that.

Any thoughts on why?

Heavy loads?  Metal fatigue?

Fred Gardner cracked his Hepburn in exactly the same way. He had it welded and it held up just fine. Not sure I would be so trusting.

I doubt that if the inside corners of the breech block opening in the receiver had a radius it would hold up any better? I know that a slight radius does help strengthen, as compared to squared when machining. Whatever did it would have more than likely cracked even with a radius in the design of the breech block opening.

If you try doing a search there is fellow and I believe his first name might be Pete and I believe he lives in Alaska who does specializes in welding up such things as you yourself now have. 
Hopefully someone will chime with his name and his contact information and he does come highly recommended for doing such work.

To Rams point I've often wondered about the square internal mortice in many single shots.  Seems a rounded corner would be stronger.  GT comments?

Is the pitting a result of corrosion or are the imperfections from casting of the receiver?

Yes, I believe a sharp internal corner like that would be a potential "stress riser" = perfect place for a crack to start.
Have a look at the mortice in a Ruger No. 1, almost looks like they pre drilled the corners with about a 3/32" drill to form a nice radius in the corners.

If I was to put money on it, that crack began when the receiver was case hardened - it's not the first I've seen.

Not safe to shoot in that condition. Kermit Hoke had one like that and had it welded and shot it for years. I don't know who welded it. I think he had it rehardened after welding.

I think if the internal corners had a very small curve it might make them stronger, but it would need to be extremely small. If there was much size to the rounded corner breech thrust would push back and the flat sides would try to expand sideways on the action.
After all the millions of various rifles made using a falling block, and this same type of machined corners to the actions, it seems like it's not really necessary to change  them. The only time they fail is when someone over stresses the action with a charge or cartridge that's way too hot.
