on my 40-60 I lost about 200 cases before I figured it out
if the case is not spotless inside and you are using poly wads..they grip the brass and stretch it causing thin spots that over a few firings crack and go down the barrel with the bullet attached
with my 38-50..3F Goex would do the same thing..went to 1 1/2 Olde E or KIK and the problem went away

I have a CPA Stevens 44 1/2 in 40/60 Maynard, the rifle is now 12 months old and has fired approximately 1000 rounds without any case problems at all. I originally purchased 50 cases from BACO which were formed from 30/40 Krag RP brass.  I'm still using those cases and so far have not had a case separation. After firing the cases are scrubbed inside with a nylon brush and then put into an ultrasonic cleaner.
My shooting procedure is to push one, DAMP, patch through the barrel and then turn it over and run it through the barrel again.
The damp patch is followed by one dry patch, I would point out however that I only use black powder which may have some bearing on case life.

This is a fairly common report and it always occurs with black powder. 
Black powder grains are hard and sharp and when fired you essentially drive a burning dirt clod out of your case. If the grains grab the case wall and the chamber is wet or oily it will just pull the end of a wimpy case off. Many of the complainers say it happens with the older WW cannelured cases and the various thin cases that result from stretching normal brass to make .40-70 SS and  .45-2.4 (45-90).
