     One of the Gerald Kelver books had a copy of the instructions included in it... I'll see if I can find my copy of the book and photocopy those two pages.  The way it decaps a case is with a spud inside like the ones on the early Ideal tong tools.  Usually they are lost by the time we find one of the tools!    After decapping, the mouth of the case is pushed onto the little conical part sticking out of one end of the tool to bell the mouth slightly.  Then the primer is replaced in the way you have already ascertained, and a measured quantity of powder is put into the case.  Finally, the bullet is placed into the expanded neck of the case and it is inserted into the die portion of the tool which is then screwed down snugly against the cartridge-to-be (with the handle spread open.) The handle is squeezed  to the tool and the cartridge is checked.  You may need to screw the die down more and clamp down again, but you are seating and taper crimping at the same time.  It's really harder to explain than it is to do!!
